Title

The scheme shall be known as FAMILY BENEFIT SCHEME and will be treated as a mutual benefit Scheme.

Head Quarters

The Head Quarters of the Scheme shall be located at FBS- Executive secretary’s office.

Date of Effective Operation

The Scheme has come into operation from 2015.

1. To provide immediate substantial financial aid to the Family of the member of the Scheme on his/her demise.

2. To provide immediate substantial financial aid to the member of the Scheme on his/her complete disability preventing them permanently to pursue dental profession.

3. To promote Life Membership / annual membership of IDA.

4. To look after the welfare of members of IDA.

5. To create oral health awareness among the general public.

Eligibility

1. Must be a Life Member/ member of good standing of IDA through local branches of IDA-A.P. State Branch (Direct memberships to ida head office are not eligible).

2. When husband and wife are both dental doctors and intend to join the Scheme they have to join as individual members in the scheme.

3. Must not have completed 50 years of age as on the date of payment of the relevant contributory fees for enrolment in the Scheme by Demand Draft. However, till 01-06-2015, all age groups are allowed to join.

4. Medically ill patients (Malignancy-primary/secondary, Serious heart problem, coronary artery disease, Bypass surgery, Angioplasty, Serious neurological diseases, Brain diseases, Serious kidney disease, Liver disease like cirrhosis, Serious Bone diseases, Degenerative diseases, Severe diabetes, Grade III hypertension) need to be declared in advance before joining. Financial aid in case of eventuality will be paid as mentioned in sub clause 5 of clause VII.

Procedure for Enrollment

1. IDA members shall need to apply in the prescribed application form which can be obtained on payment of application fee of Rs.50/- (Rupees Fifty only) (as detailed in sub clause 1 of Clause –VII) from the Hon. Secretary of the concerned Local Branch.

2. The completed application form along with the relevant Registration fee of Rs.300/-(Rupees Three hundred only) (as detailed in sub clause 2 of Clause –VII) by Demand Draft drawn in favor of “Executive secretary, Family Benefit Scheme”. Proof of Membership of IDA (Xerox copy of the receipt of Membership Subscription paid to the Local Branch / Xerox copy of Membership Certificate / copy of identity card issued by the IDA Head office ) Xerox copy of first page of S.S.C. Register or Transfer Certificate from an educational institution in which the member has studied or LIC policy or the first page of Service Register of Govt. Employees indicating the date of birth) has to be submitted to the Executive secretary of the Scheme through the Local IDA Branch Secretary with his attestation on the application form at the prescribed place.

Fees

1. Application fee Rs.50/- (Rupees Fifty only) payable once at the time of application. This amount is not refundable.

2. Registration fee Rs.300/- (Rupees Three hundred only) payable once at the time of admission. This amount is not refundable.

3. Contributory Deposit per member.

  1. ( Rs. 3000/- upto 30 Yrs )
  2. ( Rs. 4000/- 31-33Yrs )
  3. ( Rs. 5,000/- 34-36 Yrs )
  4. ( Rs. 6,000/- 37-40 Yrs )
  5. ( Rs. 8,000/- 41-45 Yrs )
  6. ( Rs. 10,000/- 46-50yrs )
  7. ( Above 50yrs - 15,000/- open only till 01-06-2015 )
  8. In case of Sub clause 4 of clause V, Members need to pay an extra contributory deposit of Rs.5,000/-.

4. Every member of the Scheme shall pay Rs.500/- (Rupees Five hundred only) each time towards Fraternity contribution in the event of death of a member and shall pay Rs.300/- (Rupees Three hundred only) in case of his/her complete disability preventing them permanently to pursue dental profession.

5. In case of clause V, sub clause-4, Rs.300/- (Rupees Three hundred only) each time towards Fraternity Contribution in the event of death of a member and shall pay Rs.200/- (Rupees Two hundred only) in case of his/her complete disability preventing them permanently to pursue dental profession which will be paid to the nominee of the deceased member and to the handicapped member.

6. A member who pays the Fraternity Contribution continuously for a period of 25 years without any break or without attracting any discrepancies in payment schedules, need not pay such contribution thereafter, but enjoys all the privileges of a regular member.

7. The Fraternity Contribution will be drawn from the contributory deposit amount of each member and paid to the nominee/s of the deceased member or to the handicapped member within 30 days of the completed claim form received in all respects. Then a demand will be made by the Executive secretary to each FBS member once in 6 months i.e., May and November each year for the outstanding dues as on that date. This amount is to be paid by all the members within 30 days of the demand.

Privileges of Members

The nominee/s of the deceased member or handicapped member of the IDA-A.P state branch - Family Benefit Scheme will be entitled to receive Fraternity Contribution benefit calculated at Rs.500/- (Rupees Five hundred only) per member in the event of death and Rs.300/- (Rupees Three hundred only) per member in case of his/her complete disability preventing them permanently to pursue dental profession, In case of sub clause-4, in clause V, the nominee/s of the deceased member or handicapped member of the IDA-A.P state branch - Family Benefit Scheme will be entitled to receive Fraternity Contribution benefit calculated at Rs.300/-(Rupees Three hundred only) each time towards Fraternity Contribution in the event of death of a member and Rs.200/- (Rupees Two hundred only) in case of his/her complete disability preventing them permanently to pursue dental profession. {Calculated by the number of Members who had registered and paid the contributory fees for the Scheme as on the first day (01) of the month in which the eventuality has occurred}.

Privileges of Members

Voluntary Retirement

If a member voluntarily retires from the membership of the Scheme, by giving a written request to the executive Secretary, his/her membership will be terminated and he/she shall get the refund of his/her contributory deposit amount as per clause VII, sub clause 3, after deduction of Rs.300/- (Rupees Three hundred only) towards office maintenance fees & if at all any dues of Fraternity Contribution as on the date of voluntary retirement.

Termination of Membership

The membership in the scheme will be terminated on the following grounds:

1. If a member furnishes wrong information in his/her application form or at any time during the period of his/ her membership by which he/she violates any provision of this scheme and thus tries to obtain any benefit under this scheme, then after giving him/her an opportunity of being heard before the executive committee and if his/her explanation is not found satisfactory, the executive committee of the Scheme shall terminate his/her membership of the scheme.

2. If a member fails to pay his/her share of Fraternity Contribution within 30 days of the demand by the executive Secretary, he/she shall be treated as a defaulter and he/she shall pay a fine of Rs.50/- per 10 days of part thereof as late fee. If a member does not clear his/ her dues within 90 days after the demand by the executive Secretary, his/her membership will be terminated and he/she will be treated as a Voluntarily Retired Member and he/she will be given such benefits as per Clause-IX of the Constitution of the Scheme.

3. If a member, at any time, for any reason ceases to be a member of IDA, his/her membership of this scheme shall automatically cease. But if such a member revives his/her membership of IDA through Ida local (AP State) / A.P. State Branch, within a period of 60 days, he/she will get his/her membership of the scheme revived on payment of revival fee Rs.100/- and arrears of Fraternity Contribution demand, if any. If he/she does not get his/her membership revived within the stipulated period, he/she will be treated as a Voluntarily Retired Member and he/she will be given such benefits as per Clause-IX of the Constitution of the Scheme. Revival by this method can be done only two times per member.

Rejoining the Scheme

After termination of the membership as per Clauses IX or X (2) and (3) the member can rejoin the Scheme as a new member by paying the application fee, Registration fee and Contributory Deposit and submitting a fresh application form, provided he/she is not barred by age limitation clause, and the arrears of Fraternity Contribution, if any, at the time of termination of membership is paid in full. However in case of termination under Sub-clause 3 of Clause–X, the member has to obtain life membership of IDA through Ida local (AP State)/ A.P. State Branch before applying for rejoining. Members who have rejoined as fresh members will have to pay Fraternity Contribution for 25 years from the date of rejoining.

Claims

(a) Procedure:

1. Immediately after eventuality (Death / Handicapped) of the member, the nominee/ member will inform the same to the Branch Secretary/ Executive secretary of the scheme and can obtain the claim form.

2. The claim form duly filled in along with the Death Certificate or Handicapped certificate and other relevant enclosures as mentioned in the claim form are to be sent to the executive Secretary of the scheme, duly attested and forwarded by the Secretary / President of the concerned Local Branch within 2 months from the date of death of the Member.

3. The First Nominee (or the Second Nominee in case of prior death of the first nominee) of the deceased member or handicapped member will get the claim as given below.

(b) Computation of Fraternity Contribution Benefit Payable :

1. The fraternity contribution is calculated according to the membership strength as on the 1st day of the month in which the death has occurred at the rate of Rs.500/- (Rupees Five hundred only) in the event of death and Rs.300/- (Rupees Three hundred only) in case of his/her complete disability preventing them permanently to pursue dental profession less dues of Fraternity Contribution, if any, from the member at the time of his/her death or complete disability which will be paid to the Nominee/s of the deceased member and to the handicapped member.

2. In case of sub clause-4, in clause V, Rs.300/- (Rupees Three hundred only) each time towards Fraternity Contribution in the event of death of a member and shall pay Rs.200/- (Rupees Two hundred only) in case of his/her complete disability preventing them permanently to pursue dental profession less dues of Fraternity Contribution, if any, from the member at the time of his/her death or complete disability which will be paid to the nominee/s of the deceased member and to the handicapped member.

Management of the Scheme

The management of the Scheme vests with the IDA - A.P. STATE BRANCH - FAMILY BENEFIT SCHEME committee.

IDA A.P. Stae Branch - FBS Managing Committee

The management of the Scheme vests with the IDA - A.P. STATE BRANCH - FAMILY BENEFIT SCHEME committee.

(a) Composition :

The Management committee of the Scheme consists of Ex-officio and Elected Members.

Ex-Officio Members :
  • 1. President of IDA-A.P. State Branch.
  • 2. Hon. State Secretary of IDA-A.P. State Branch.
  • 3. Treasurer of IDA-A.P. State Branch.
  • 4. Immediate past executive Secretary of the IDA -AP State branch-FBS scheme.
Elected Members :
  • 1. Executive Secretary (Shall not be an office bearer of IDA AP State).
  • 2. Treasurer (Shall not be an office bearer of IDA AP State).
  • 3. 5 members representing from FBS council.
(b) Functions :

1. The managing committee shall be overall in-charge for the management of the scheme.

2. It shall receive, discuss approve the reports and accounts submitted by the executive secretary & treasurer for the period between two consecutive meetings of the managing committee.

3. The managing committee in consultation with FBS council shall decide the policy regarding disbursement and investment of funds at the disposal of the scheme as fixed deposits as mentioned in Sub-Clause 7 of Clause –XVIII.

(c) Meetings :

1. The managing committee shall meet once in every 3 months preferably at the EC meetings of IDA AP State Branch, out of which one will be held immediately preceding the meeting of IDA AP State AGM.

2. The quorum of Managing Committee meeting shall be six of whom at least three are elected members for FBS.

3. If a member fails to attend two consecutive meetings of the Managing Committee without giving prior intimation in writing to the executive Secretary of the scheme indicating valid reasons for the absence, he/she will automatically cease to be a member of Managing Committee from the date of the second meeting. The FBS Council shall elect a new member.

4. The elected Members & Ex-officio members of the FBS shall have voting right in the Managing Committee Meetings.

5. The notice of the meeting of the Managing Committee shall be sent 12 days before the date of the meeting.

6. Emergency Meeting: The executive Secretary of FBS, shall, in consultation with the president & secretary of IDA AP state, convene an emergency meeting of Managing Committee to transact any urgent business with a notice of one week.

(d) Elections to Managing Committee:

1. For the first term, The FBS body will appointed by the AP State EC, From second term, All the elected members including executive secretary & Treasurer of the Managing Committee of the Scheme shall be elected by the FBS Council formed from among the members of FBS.

2. The executive secretary & Treasurer shall reside at the same place and shall be the place of Head Quarters of the Scheme.

3. The Executive Secretary shall invite nominations for all posts at least 30 days before schedule meeting of the FBS council. The nominations should reach the Secretary of the scheme by the date fixed by him in consultation with FBS council, which shall be at least 10 days before the FBS council meeting. Any withdrawals can be made on the floor of the house provided the candidates are physically present or their written consent is obtained in advance. The above election will be held at the FBS Council Meeting

Tenure of Office:

The term of office of executive secretary, Treasurer & 5 members representing FBS council will be for 3 years.

IDA A.P. State Branch - FBS Council :

(a) Composition :

1. It shall comprise of members elected from among the FBS members of each local branch of IDA in the following proportion 1 to 100 members - 1 representative. More than 100members one additional representative only.

2. The election for FBS council shall be held along with the AGM of the local branches. All the Managing Committee members shall be Ex-Officio members of the FBS Council.

(b) Functions :

1. The FBS Council shall be a representative body of the General Body of the FBS.

2. The FBS Council shall make suggestions to the managing committee for the management of the Scheme.

3. It shall elect the members of managing committee.

4. It shall carry out any amendments the Constitution as required form time to time.

(c) Meetings :

1. The Annual Meeting of the FBS Council shall be held at the same venue of the AGM of IDA-AP state every year.

2. Notice of Annual Meeting of FBS Council will be given at least 21 days in advance notifying the date, place and time of the meeting with the agenda.

3. The quorum of FBS Council meeting shall be 30% of the representatives in the Council or 30 whichever is less, of whom at least 20% shall be elected members.

4. In case there is no quorum, the meeting shall be adjourned and reconvened to transact the business on the agenda only.

5. The FBS Council meeting shall take decisions by a simple majority of the members present except decisions pertaining to changes to the constitution which shall be decided by two third majority of the members present.

6. Requisition Meeting of FBS Council shall be called by a requisition signed by 20% of the representatives of FBS Council on the register on that day. The meeting shall be convened within thirty days from the date of receipt of the requisition. The date, time and venue can be decided by the Executive Secretary of the Scheme in consultation with the IDA AP State President & Secretary.

7. Emergency meeting of FBS Council : The executive Secretary of FBS shall, in consultation with the IDA AP State President & Secretary, convene an emergency meeting of FBS Council to transact any urgent business with a notice of one week.

(d) Elections to FBS Council :

The representatives from Local Branches to the FBS Council shall be elected by the local branches in the AGM of the local branches every year and their names with full postal addresses shall be communicated to the FBS Head Quarters Office latest by 30th of December every year. In case of no communication being received by the specified date from the branch, The secretary shall be the FBS council member till the intimation is done from the local branch.

General Body :

(a) Composition :

1. The General Body consists of all the members of the Scheme.

2. It shall be the supreme authority of the Scheme.

(b) Functions :

The General Body shall meet only in extraordinary circumstances and the meeting shall be called with a notice of 21 days in advance.

(c) Meetings :

1. The quorum for the meeting shall be 20% of the members of the scheme or 50 members whichever is less. If there is no quorum, the meeting shall be adjourned and subsequently convened to transact the business on the agenda. The General Body shall take decisions by 2/3 majority of the members present.

2. Requisition meeting of the General Body shall be called for by a requisition signed by 20% of the members of the scheme on the register as on that date or by 100 members whichever is less. If there is no quorum at such a meeting the same shall be deemed to be dissolved for that requisition

Procedure for Amendments to Constitution :

1. The FBS Council members before 15th October each year can send to executive Secretary of the scheme asking for any resolutions / amendments to the constitution to be placed on the agenda of the annual meeting of FBS Council. The FBS Council members shall inform the same to all the primary members of FBS in their branches. Copies of the circular will also be marked to the President and Hon. Secretary of the local Branches.

2. The resolutions/ amendments to the constitution of the FBS duly proposed and seconded by two individual members of the scheme shall be sent to the FBS Office latest by 30th October of the year. Any resolution received after this date will not be considered valid.

3. The resolution/amendments to constitution have to be duly circulated to all the members of the FBS Council along with the notice of the FBS Council meeting.

4. The resolutions / amendments to the constitution have to be considered by the Managing Committee at its meeting preceding the annual meeting of the FBS Council for its recommendation to the Council.

5. The resolutions / amendments of the constitution shall be considered and accepted as passed provided two thirds of the members of the FBS Council present, and voting at the meeting, vote in favor of the resolutions / amendments to the Constitution.

6. The amendment passed by the FBS Council shall take effect from the date notified by the council.

Accounts and Audit :

1. Separate Bank Accounts shall be opened in any Nationalized Bank / Urban co-operative Bank of good reputation. The accounts shall be operated jointly by executive Secretary & Treasurer.

2. The financial year of the scheme shall be form 1stJanuary to December 31st.

3. The Managing Committee shall approve the accounts submitted the Treasurer duly audited by the Charted Accountant as also the budget for the succeeding year.

4. The accounts shall audit every quarter by internal audit.

5. Audited accounts of the scheme and the budget for the succeeding year passed by the Managing Committee shall be ratified by the FBS Council at its annual meeting.

6. The ultimate authority for investment, utilization and disbursement of the funds shall entirely vest with the Managing Committee of the Scheme.

7. The funds of the scheme shall be deposited in Nationalized Bank/ National Savings Certificates and Urban Co-Operative Banks of good reputation giving good interest with adequate security. The Executive Secretary and Treasurer will jointly operate the accounts. Any withdrawal of above Rs.15000/- at a timeshould be supported by a resolution of the Managing Committee obtained, if necessary, by postal circulation.

Duties and Privileges of Office Bearers :

1. President - IDA A.P. State (Chairman) : The Chairman of the scheme shall preside over all the meetings of the Managing Committee, FBS Council and General Body. In the absence of the Chairman, The Vice-Chairman shall preside, The Chairman shall have a casting vote in addition to his regular vote in case of there being a tie of votes whenever there is voting either in the Managing Committee or FBS Council or General Body. The Chairman is the executive head of the Scheme.

2. Hon. General Secretary (Vice Chairman) : In the absence of the Chairman, the Vice-Chairman shall preside over all the meetings of the Managing Committee, FBS Council and General Body.

3. In absence of President & Hon. Gen. Secretary-AP State, a Chairman shall be elected from amongst the members present.

4.Executive Secretary : The Secretary shall carry on the day to day functions of the scheme and shall implement the decisions taken by the Managing Committee. He shall be overall in-charge of the membership enrolment, fees collections and routine administration besides other duties and functions that are specified by the Managing Committee. He shall be assisted by the Treasurer in discharging his duties.

5. Treasurer: The Treasurer shall look after maintenance of the day to day accounts of the scheme, and submit the accounts for approval to the Managing Committee and FBS Council. He shall submit a statement of Audited Accounts and the budget estimates for succeeding year annually to the Managing Committee and FBS Council at the annual meeting for approval. He shall act for the Executive Secretary during the absence of the latter. He shall also submit statement of accounts to the meetings of the Managing Committee.

Travelling Allowance & Conveyance Allowance to Office Bearers :

1. The executive secretary & Treasurer shall be paid Rs.500/- per meeting as Travelling & conveyance allowance from FBS office (if meeting held at places away from the FBS office).

2. This amount shall be Rs.500/- for members of Managing Committee and shall be paid from respective local branch / state branch (President, Secretary, Treasurer).

Miscellaneous :

1. Every member shall specify his/ her 3 nominees for obtaining benefits under this scheme.

2. Any income derived from the property of this scheme is utilized only for the objectives of this scheme as per clause IV. Under no circumstances the FBS funds can be diverted for any other purposes.

3. The executive secretary of the scheme will submit periodical reports of FBS at the meeting of the State EC meeting.

4. Any suggestions / recommendations shall be discussed by the Managing Committee which will pass on the same with its recommendations to FBS Council, which shall take the final decision.

Dissolution :

If it becomes impossible to carry out the objectives of the scheme, the scheme shall be dissolved by a decision taken at an extra-ordinary General Body meeting of the members of the scheme convened by the executive Secretary and attended by at least 20% of members on rolls as on the day of the notice, decided by the vote of ¾ of the members who attended the meeting, and another such extra-ordinary General Body Meeting of the members of the scheme shall there upon decide the final disbursement of the corpus fund after meeting the liabilities and debts and recovering the assets keeping in view, the objectives of the scheme, that is mutual benefit and charitable purpose, to the members concerned in proportion to their contribution to the scheme and this decision taken by the extraordinary General Body Meeting shall be final, legal and non-negotiable.
